Even before the pandemic of 2020, anxiety was crippling the performance of workers—especially young employees. Through research Chester Elton learned how a few vital leadership skills—when done right—can boost employee engagement and reduce turnover. He shares the fascinating science behind why so many managers resist something so seemingly simple as gratitude and offers 8 tactics for managers to help their employees build resilience. Each attendee will receive a copy of two of Chester’s books: Anxiety at Work: Eight Strategies to Help Teams Build Resilience, Handle Uncertainty, and Get Stuff Done and Leading with Gratitude: Eight Leadership Practices for Extraordinary Business Results written with Adrian Bostick.  Breakfast is included and starts at 8am; the session will start at 9am.Read More

CLICK HERE TO VIEW SESSION RECORDING. Home Health, Hospice, and Home Care are about helping people helping people. We have been focused on improving lives of patients and families since the industry's inception. The pandemic changed the Home Care/Hospice industry in many ways. Home Care now sits in the front row of the healthcare continuum. We not only compete for referrals but must compete for talented clinicians to support the abundance of patients who need our services. This presentation will combine market data, hiring trends, and retention statistics with tangible tactics and real-life examples of how leading agencies are winning the competition for people. Led by two industry-leaders on human capital from both the consultant and provider points of view, Eric Scharber is a national expert on recruitment, retention, and staffing trends in Home Care; Brent Korte has deployed innovative tactics for one of the largest providers in the US for years, resulting in some of the lowest turnover rates in Home Care.Read More

Home-based care providers have been challenged to employ lean principles and create a staff-driven culture because of a dispersed care settings and a remote workforce. For one Home Health, Hospice, and Home Infusion program, a quality and organizational culture system built on a commitment to zero harm model is helping them reduce all harms and simultaneously increase staff retention. The fundamental goal of their model called Commit to Zero Harm is real-time problem solving to manage risk of harm to patients, their loved ones, and each other. Components of the program include 15-minute tiered huddles, escalation pathways, intense leadership engagement, and shared learnings. This agency is now seeing significant improvements in key safety and performance metrics. Some areas of improvement include increased retention rates up to fifty percent, more timely patient access to care, and other safety performance outcomes.Read More

Home health outcome measured have historically focused on the ability of agencies to demonstrate improvement as seen in the scores that are publicly accessible and the related stars ratings. Although there is a shift to some composite measures in the value based purchasing model, agencies continue to see improvement as a “positive” outcome and stabilization or decline as “negative”. Discussion of the model in the final rule for 2022 highlighted the importance of both maintenance therapy and nursing management and evaluation - two areas that make agencies nervous and leading to both components of the home health benefit being underutilized. This session will connect the stabilization approach to care to positive patient outcomes as home health value based purchasing continues to expand.Read More

Varied payment models between Medicare, MA and other fee-for-service or value-based payers can make it difficult to answer questions, such as: “Is my revenue right?,” “Why are my receivables so high?,” and “Where is my cash?” It can often be difficult to make sense of changes in revenues, receivables, and cash flow when patient census and utilization do not seem to vary. This session will provide attendees with strategies for assessing how to identify subtle changes in operations that can have big impacts on revenue performance and cash flow, such as changes in coding practices, shifting volumes between PDGM and fee-for-service payers, and delays in order management. This session will also explore how varied or limited use of EMR systems can contribute to growing receivables or financial reporting inaccuracies. Attendees will be provided insights on how to analyze and improve revenue cycle performance and reporting.Read More

During these incredibly difficult years, we have found ways to use this challenging time to step up and show our employees we know they are our top priority. With healthcare professionals continuing to undervalue the stories that lie behind every patient, we only continue to fail in providing better patient outcomes. Hospital readmission rates and emergency room visits remain high, requiring CMS to take further action in incentivizing agencies thru Value Based Purchasing. Agencies, no doubt, heal wounds and maximize potential, but what happens after discharge? Why are we finding these patients back at our door? Patients are people, like you and me. We belong to a certain demographic that influence how we live. We encounter real-life challenges that already make life difficult as it is. But some of these pose as obstacles to our health. A person with CHF who is short of breath may not have the ability to prepare a low sodium meal. Their priority of living is greater than a low sodium diet, which could mean that TV dinners becoming staple in that household. Life happens at home. As it continues to expand exponentially and moves to digitally enabled healthcare services, cyber-criminals are trying to take advantage of the weaknesses and security vulnerabilities. With the global pandemic, healthcare organizations continue to fall victim to data leaks involving compromise on privacy, personal information, and sensitive health records maliciously being sold or distributed on the Dark Web. Educating your staff and establishing a strong IT foundation can help safeguard your client’s privacy and personal information. Agencies need to work closely with their software vendors to ensure that their business is properly protected.Read More

CLICK HERE TO VIEW SESSION RECORDING. This session will cover the details of the Value-Based Insurance Design (VBID) Model – Hospice Benefit Component. Medicare Advantage (MA) has long had coverage for home health patients, but there has never been coverage for Hospice under the MA benefit. VBID is a demonstration that began January 1, 2021 and continues for 5 years. There are multiple Medicare Advantage Organizations (MAO) that are participating and have coverage in multiple states. In addition to Hospice care there is an expansion of Palliative Care services that are offered to patients through the MAO. CMS is testing how the hospice benefit component can improve beneficiary care through greater care coordination, reduced fragmentation, and transparency in line with recommendations by the Office of Inspector General (OIG), the Medicare Payment Advisory Commission (MedPAC) and others. Hospice must prepare as more and more MAOs are participating in VBID to offer the hospice benefit to their beneficiaries.Read More

The early days of the coronavirus pandemic sent many long-term care facilities into lockdown and families were faced with the decision of being separated from their loved ones or finding ways to transition them home. The call for private duty agencies to fill this gap became apparent, however, many agencies were not prepared with specialty programs or trained caregivers to facilitate the transfer of care to the home environment. This presentation will prepare private duty operators and owners with the knowledge they need to develop a transitional care program that will be a value service to hospitals, rehab facilities, long-term care facilities, payer sources and most importantly, clients and families. Grow your business quickly by putting this service line in place and improve your caregiver retention rates while elevating the need for private duty care across the continuum.Read More

They can be complex & multidimensional, but a worthwhile endeavor. Decision makers need reliable and valid ways to measure the cost saving & leaders need to understand the good work and progress already created to measure the impact of any new initiatives. Once a decision maker identifies areas where a business should reduce costs, it’s important to develop principles to improve consistency & reliability for optimum success. Leaders need to effectively implement these strategies to an organization, knowing that with any change, there will be ongoing conversations & other changes that take place as a result. This session will discuss identifying top cost saving benchmarks for home health and hospice organizations, describe cost reduction initiatives to and how to strategize implementing them to positively impact client & employee satisfaction while improving the bottom line.Read More

The 4Ms framework for developing Age-Friendly Care is an innovation in care that directly challenges ageism and serves to promote the well-being and quality of life for older adults. Reframing attitudes and approaches towards aging and older people are critical during this time of unprecedented longevity. Identifying and addressing strategies to combat ageism in home health, hospice and caregiving though the application of Age-Friendly Care is not just a clinical opportunity, but a leadership priority towards overall organizational success. This proposal aims to equip organizations with age-friendly tools and strategies to optimize their approach in communications, marketing, clinical care, and training of staff towards a burgeoning older clientele.Read More

The resources consumed by the QA process can be a financial strain on the organization not to mention to time involved with moving the document back and forth. Focusing almost exclusively on the speed of the QA process is insufficient and may actually drive up costs. OASIS E provides an opportunity to reframe the conversation and preparation must include revisiting the review process to determine if change is needed. This session will address the three pillars that impact achievement of a “clean” OASIS: ● Information collection by Intake ● Data Collection by Clinicians ● Accountability of Clinicians and QARead More

To ensure optimal care, a pharmacist led interdisciplinary team conducted a quality improvement project at a non-profit home healthcare agency focusing on improving medication reconciliation and evaluation in patients at high risk for hospitalization. The project connected an improvement in medication reconciliation to a reduction in 30-day hospitalizations in patients with heart failure. Prior to the project, the baseline median for 30-day hospitalizations was 20.0%; following full implementation, this dropped 9.1% among patients with heart failure. This presentation describes the development of the interdisciplinary quality improvement team, the approach of the team, the selection of the metrics, the overall results, and future directions for further collaboration between home healthcare agencies and pharmacists.Read More

CLICK HERE TO VIEW SESSION RECORDING. Everyone is talking about it – the workforce shortage, the caregiver crisis, the need for, now and in years to come, more caregivers. Private home care providers were acutely aware of this long before the pandemic punctuated it across all industries. And, while there is no silver bullet solution, we can combat this. Right now, public perception of the role of a caregiver, home care aide or PCA is pretty poor – home care workers are thought of as unskilled or poorly educated. Culturally, the US doesn’t support choosing caregiving as a career – starkly different than many other countries where many generations of families live together and care for their elders. Join a panel of experts as we take a deep dive into the current perception of the home care worker and what we can do to change it. Read More

CLICK HERE TO VIEW SESSION RECORDING. The United States healthcare industry has a people problem. There just are not enough available caregivers to meet the needs of the public. When people are faced with new medical problems, or when they have chronic health issues they turn to the expertise, counsel, and compassion of nurses, aides, doctors, and a whole cadre of other health professionals. From 2010 to 2050, the United States population ages 85 and older is roughly expected to triple from under six million to over 18 million. Without significant reforms and innovation, there will not be enough caregivers to support the needs. In this outline we suggest strategies to counter the workforce crisis and the policies that need to be adopted to make systemic change to fix our people problem.Read More

Unfortunately, some entrenched team meeting practices miss the mark on time and resource management, while losing sight of best practice objectives for utilization, compliance, and outcome improvement. When the home health case conference/IDT meeting evolves beyond these old habits and uses risk-informed, data-driven best practices, it holds the opportunity to stack clinical skills, advance QAPI initiatives, strengthen compliance, and revitalize engagement. Our home health clinicians, as well as their leaders, are braving a new world of performance challenges within workforce constraints, TPE resumption, PDGM pressure, the pandemic, and HHVBP. Build strength in adopting new ways to approach these challenges with your teams! Patients/clients receiving care at home are typically accustomed to receiving high-level service, and they want to maintain a personal relationship with the agency providing the care. Home care agencies need to adopt marketing strategies at the grassroots level, and this presentation explains how to leverage community-based initiatives that will plant the seed for future growth. The best part is that grassroots programs are usually free or very inexpensive as you only have to invest your time! The presentation will help you plan out how to identify these community targets, develop a strategy, implement the initiative, and continue to plant the seed for future growth.Read More

However, by knitting together the right partners, agencies can add critical tools to their care delivery toolbox. The largest providers keep getting larger and transaction values have risen to dizzying heights. The biggest deals get most of the attention, but the industry is still highly fragmented and there is still very strong demand for small and mid-sized providers of every type. In this session we’ll discuss who’s buying and how to best position your agency to appeal to the buyers paying the highest premiums.

While finding skilled staff has become an insurmountable challenge, administrative staff face their fair share of difficulties too. Regulations and compliances have created an administrative burden on staff that results in teams spending too much time on repetitive work. In this session, we address the role of Robotic Process Automation (RPA), an innovative technology in post-acute care. Learn how RPA helps home-health and hospice teams create automated workflows that function like a digital assistant, performing repetitive, logic driven administrative work. At the end of the session, you will witness how teams are able to save time as well as costs on additional resources, and focus on improving the quality of care delivered. For organizations burdened by the staffing shortage, this session is an insightful view into the technology that can alleviate this problem.Read More

A pandemic, three vaccines, a new administration, and trillions of dollars of new federal spending later, where is the market today, and where is it heading? Why did hospice valuations rise to unprecedented levels, and what could rein them in? How did PDGM part one and part deux impact home health deal flow, and what can we expect heading into 2022? How will strains on state budgets impact Medicaid reimbursement and eligibility? What impact will PTSD born shortages of nurse staffing have on providers? Are the reasons driving consolidation pre-COVID still relevant? Effective communication with patients and families, via the right channels, at the right time, is a powerful driver of client satisfaction and positive care outcomes. However, the time commitment involved in recurring check-ins, schedule coordination with clients and caregivers, responses to follow-up questions raised during visits, monitoring medication, and securing physician authorizations all add pressure on already overstretched staff. This session will share proven technologies your agency can deploy to increase the flow of important information. Presented within the context of actual agency workflows, the session will target integrated strategies enabled through technologies such as HIPAA-compliant texting, AI-powered chatbots delivering secure messaging through text links, and effective use of telehealth/virtual visits. With the impacts of the pandemic on your organization’s culture, morale, recruitment, retention, and financial outcomes - all combined with the everchanging healthcare regulatory and reimbursement landscape - the future can look daunting. The impacts of the national roll out of value-based purchasing (VBP), the sustained growth of Medicare Advantage (MA), the reduction of Medicare Fee-For-Service (FFS), and the recent addition of the Value-based Insurance Design Model (VBID) can be tamed through the effective use of the strategic process improvements. In this session you will gain perspectives on how the strategic process works, how it can prepare you for these changes and the ones to come, but, most of all, how to roll these initiatives out to your staff to assure your organization thrives given the stresses of the operating environment today along with the rise of these transformational reimbursement models.Read More

States have to maintain a level of compliance when it comes to CMS certification of their EVV program. This means using EVV data to determine if claims should be paid. Whether your state has already turned on the denials, or if they’re just preparing for it, you have to make sure your agency can survive. Some of the questions to ask yourself: If offered, should you use the state solution? Are your state solution and other admin accounts set up for auditing EVV submissions and billing for compliance issues? If you’re using your own vendor, are they ready and configuring the system correctly? How do you plan to educate field staff to promote buy in? While over 30% of patients in home health have a wound, providing standardized, best practice care is a challenge. In 2020, a home health agency that provides services across five states, launched a nurse-led, remote-enabled wound care program with their innovative technology partner. To underpin this new, virtual wound care program, the nursing team adopted an AI-powered, digital wound care platform to allow frontline clinicians to easily capture high precision wound images in the home and then share this data with the remote wound experts to augment clinical decision making to drive more preventive care. A benefits study of the program revealed significant improvement after only 6 months: - Reduction in 60-day rehospitalization rate. - Reduction in incidence of new wounds in home health. - Improvement in wound healing rate. - Reduction in documentation time. - Reduction in nursing visits per wound episode.Read More

In a healthcare environment where home care and hospice are increasingly judged on care quality, it’s critical to have a full clinical history of your patients. Too often, organizations are limited to the data they collect themselves. Gaps in data mean you can’t know exactly how ill a patient may be and when to transition them from home care to hospice. You can’t know all the providers and facilities involved in the patient’s care. You can’t know which patients are at risk of high acute utilization. You can’t know when patients are being billed for services outside the Hospice Benefit. And you can’t track the quality metrics important to prospective referral partners and payers. This presentation will introduce the new CMS pilot program, Data at the Point of Care (DPC), which provides 3+ years of CMS Part A, B, and D data, and review how this data can be integrated with your EHR and used to coordinate care between providers, improve care quality, and help your organization thrive.Read More

What constitutes an eligible hospice diagnosis? What are related conditions and relevant comorbidities? What are hospice coverage responsibilities? This session will provide a simple algorithm for determining hospice eligibility and outline an approach for identifying eligible related conditions, including specific regulatory directives. It will also provide guidance for accepting or rejecting specific comorbidities, diseases, and conditions as related conditions. Also discussed will be the financial implications of accepting or rejecting related conditions.
