Description
Session attendees will understand and identify home care’s unique and vital role in addressing health disparities. HCA E&R sought out grant funding and multiple partners to develop the initiative, Addressing Health Disparities Through Home Care. As a part of this initiative, a needs assessment was conducted to identify interventions and best practices that can help eliminate persistent gaps in care. To address home care and hospice needs, multiple initiatives were developed. Education and training to support staff development and training of cultural competencies, the enhancement of their ability to treat and care for patients with co-occurring physical and mental health conditions, and certifications of clinicians at a sample set of agencies in population care coordination. Additional components include a translation initiative, a new health literacy program, and a collaborative pilot model. When disparities are recognized and addressed, the patient and the community benefit.
