Description

Researchers investigated the prevalence of emergency department (ED) utilization by focusing on primary care provider (PCP) relationship support use by home health patients. This session provides a review of reasons why home health patients used the ED, coupled with preventable and non-preventable analysis. We will discuss when the home health patients could not wait for a home health visit, and the percent of patients using the on-call option versus following nurse and physician guidance to go immediately to the ED. This session will discuss the research findings, and which patients were admitted to the hospital from their ED visit. The researchers will identify opportunities for changing home health care options to better support the population of patients seeking assistance in an ED. The presenters will identify how PCP relationships are impacting home health patients and clinicians, payors, and providers.
