Description

This educational session aims to enhance the knowledge and skills of home health professionals in maximizing the utilization of home health visits to achieve positive patient outcomes. Participants will learn strategies to improve patient engagement, promote adherence to care plans, and facilitate effective communication between patients, caregivers, and healthcare providers. This session will include interactive case studies and group discussions to apply the concepts learned to real-life scenarios. At the end of the session, participants will have the knowledge and skills to maximize the utilization of home health visits to achieve positive patient outcomes.
