Description
Post Covid-19 leaders recognize that meeting patient needs and increasing service demand would require changes, allowing clinicians to practice at the top of their license. Many home visit issues distracted clinicians from patient care and could be done virtually by other roles. Timely initiation of care and job satisfaction by clinicians was necessary. Administrative tasks were time-consuming and unnecessary tasks performed by clinicians. We created a new care delivery model allowing the patient to be the center of the care team. Case management and quality care were developed into central team roles to support field clinicians, allowing them to have satisfying patient care visits.
