Description

All organizations are seeking creative ways to provide the right level of care at the right time to ensure quality outcomes and patient satisfaction. Additionally, it is increasingly important to coordinate programs longitudinally to better manage total cost of care and reduce avoidable hospital readmissions. This is true in risk-based programs with payors or to meet the demands of health systems. The ability to provide multiple levels of care in the home is central to satisfying all stakeholders. This session explores how several organizations are addressing the opportunity through joint ventures or other structures – including personal care, home health, hospice, palliative care, SNF at home, hospital at home, or some combination of these programs.
